The most common reason an error is received and a document cannot be replaced is that the original file was uploaded with a special character in the filename. Even if renamed later, the original file path generated with any special character except an underscore (i.e. hyphen/dash, slash, or period) will corrupt the file path so that it cannot later be updated.
