Force Login requires users to log in to view a form.

Information submitted to any form fields will be transmitted via email for administrator notification and thus is not eligible to be adequately protected sensitive information. For this reason, do not request the following information on any forms:

• Bank Account Information
• Birth Certificate Copy
• Business ID Numbers
• Driver’s License Numbers
• Medical Information
• Passport Numbers
• Passwords
• Routing Numbers
• Social Security Numbers
• Tax ID Numbers
• W2 or Other Tax Information

Instructions

1. Navigate to Modules > Content > Form Center
   
   
2. Select your desired category
   
3. Select Actions next to the desired form
   
4. Select Modify from the drop-down menu
   
   
   • Note: If you want to create a new form, skip this step.
   • Note: Authors only see this option on forms they have saved or submitted.
5. Click Show Additional Details
   
6. Click the Force Login checkbox
   
7. Select Save
