You can use a template to build a form, which is easier and faster than building one from scratch. Our CivicPlus Sample Forms Category makes it easy to find a form that you can copy and modify to fit your needs.

Instructions

1. Navigate to Modules > Content > Forms 
   
   
2. Select Choose an Action > View List next to CivicPlus Sample Forms
   
   
3. To the right of each form name, you can select Preview or Copy from the Choose an Action drop-down menu
   
   
   • Preview: Will open the template in a new window; allows you to find the template that collects the information you want
   • Copy: Will bring up the template properties
